I had the same conversation hundreds of times
Before I started VibeSec Advisory, I spent years as a sales engineer helping enterprises adopt AI tools. I talked to CTOs, VPs of Engineering, product leaders, and security teams at companies ranging from 50-person startups to Fortune 500 organizations.
The conversations were remarkably similar. The same concerns. The same mistakes. The same patterns of companies that moved fast and companies that got stuck. After a while, I could predict within the first five minutes of a call whether a company would successfully adopt AI tools or spend the next 18 months in analysis paralysis.
Here is what I learned.
Pattern 1: They wait for permission that is not coming
The most common pattern I saw was companies waiting for someone else to tell them it was safe to adopt AI tools. Waiting for legal to approve it. Waiting for security to review it. Waiting for the CEO to declare an AI strategy. Waiting for a vendor to promise zero risk.
That permission never arrives, because there is no such thing as zero-risk AI adoption. There is only managed risk.
The companies that moved fast did not wait. They started with a small team, picked one AI tool, set clear boundaries on what data it could access, and learned by doing. They created their own permission structure instead of waiting for one to appear from above.
What to do instead: Pick one AI coding tool. Deploy it to one team. Set three rules: no production data in prompts, no AI-generated code in security-critical paths without review, report anything unexpected. Learn from that before trying to roll it out company-wide.
Pattern 2: The security team says no to everything
I watched security teams kill AI adoption at dozens of companies. Not because the risks were unmanageable, but because the default posture was "no" and nobody had a framework for getting to "yes."
The conversation usually went like this:
Engineering: "We want to use Cursor." Security: "What data does it send to the model?" Engineering: "Code context from open files." Security: "That could include secrets, PII, proprietary logic. Denied."
End of conversation. Engineering uses it anyway on personal laptops. Shadow AI grows. The security team has no visibility into what is actually happening.
The companies that got this right had a security team that said "yes, with guardrails" instead of just "no." This is the Guardrails pillar in practice. Guardrails are not bans. They are constraints that define what an agent or tool can access, where data is allowed to go, and what happens when something goes wrong. A guardrail is: "Cursor can access files in this repo, not these directories, and never in a terminal with production credentials loaded." That is a manageable constraint. "No AI tools" is not a guardrail. It is a gap waiting to be worked around.
They created an AI tool approval process with clear criteria: what data can the tool access, where does the data go, what controls exist, what is the blast radius if something goes wrong.
What to do instead: Create a one-page AI tool evaluation template. Three sections: data exposure (what data does the tool see?), controls (what guardrails exist?), and risk acceptance (who signs off?). Make the approval process take days, not months.
Pattern 3: They buy a tool instead of building a process
The number of companies that asked me "which AI tool should we buy?" when they should have been asking "how should we integrate AI into our development workflow?" was staggering.
A tool is a thing you install. A process is how your team works. AI adoption is a process change, not a procurement decision. They needed a process design methodology. They needed to define the Skills their AI would perform, the Agents responsible for each step, the Guardrails that would keep things in bounds (including where humans would review and approve), and the Schedule that would govern when and how often those processes ran. They bought a license instead.
The tool matters less than:
- How developers are trained to use it
- What code review looks like for AI-generated code
- How you handle the security implications
- Who is responsible when AI-generated code causes an incident
- How you measure whether AI is actually making your team faster
I watched companies buy $100K enterprise AI licenses and get less value than a startup that gave every developer a $20/month Cursor subscription and spent an afternoon writing three rules for how to use it.

What to do instead: Before buying any AI tool, answer these questions: Who will use it? For what tasks? With what data? Under what constraints? Who reviews the output? Write the answers down. That document is your AI adoption process. The tool selection comes after.
Pattern 4: Nobody owns AI governance
At most companies I talked to, AI governance was everybody's problem and nobody's job. Engineering owned the tools. Legal owned the risk. Security owned the threats. HR owned the acceptable use policy. IT owned the procurement. Nobody owned the complete picture.
The result was predictable: each department made decisions in isolation. Engineering approved a tool that security had not reviewed. Legal wrote a policy that engineering could not follow. Security blocked a tool that the CEO had already announced externally.
The companies that moved fast had one person (or one small team) who owned the cross-functional AI strategy. Not a committee. Not a steering group. One person with the authority to make decisions and the mandate to coordinate across departments.
What I eventually realized is that governance needs a structure, not just an owner. The FORGE Methodology framework gives it one: Skills tells you what your AI can do, Agents tells you who is responsible for each step, Guardrails tells you what the constraints are (including where humans approve), and Schedule tells you when and how often each process runs. When governance has those four things defined, "one person owns AI" becomes executable. Without that structure, even a dedicated AI owner ends up spinning their wheels coordinating departments that are all speaking different languages.
What to do instead: Name an AI owner. This person does not need to be a full-time hire (for most companies, it should not be). It can be the CTO, a senior engineering manager, or an outside advisor. What matters is that one person has the authority to make AI decisions and the responsibility to coordinate with legal, security, and engineering. Give that person a framework, not just a title.
Pattern 5: They treat AI-generated code like human-written code
This one was almost universal, and it is the mistake that creates the most security risk.
AI-generated code looks like human-written code. It follows conventions. It uses reasonable variable names. It compiles, passes tests, and often works correctly. This creates a false sense of confidence.
But AI-generated code has a fundamentally different risk profile:
It optimizes for working, not for secure. An AI model produces code that satisfies the prompt. Security controls are added only when explicitly requested or when the training data included security-first examples. Rate limiting, input validation, proper error handling, secure session management are all things the developer has to ask for.
It introduces vulnerabilities at scale. A human developer writing one insecure endpoint is a one-off mistake. An AI generating the same insecure pattern across 50 endpoints is a systemic vulnerability. The speed advantage of AI coding is also a vulnerability amplifier.
Nobody reads it closely. This is the big one. When a developer writes code by hand, they understand every line because they wrote it. When AI generates 200 lines of code and it works, the developer accepts it and moves on. The code is functionally correct but has not been reviewed for security implications.
This is a Guardrails problem. The guardrail is not "do not use AI for code." The guardrail is: "AI-generated code is untreated input until a human reviews it for security." That is a constraint you can build into your process. Every AI-generated code block goes through the same review as a pull request from a new contractor. The review is the guardrail.
What to do instead: Treat AI-generated code as untrusted input. Create a checklist: Does it validate inputs? Does it handle errors securely? Does it expose sensitive data in responses? Are there hardcoded secrets? Is authentication and authorization properly implemented?
Pattern 6: The "we will do security later" trap
I heard this in probably half of my conversations: "We know we need to address AI security, but right now we are focused on adoption. We will circle back to security in Q3."
Q3 never comes. Or when it does, the codebase has six months of AI-generated code with zero security review, the team has adopted three more AI tools without evaluation, and the security debt is so large that nobody wants to look at it.
Security is not a phase. It is a constraint that should be applied from day one of AI adoption. Not "we will lock everything down" levels of constraint. Just basic hygiene:
- Do not send production data to AI models
- Review AI-generated code for security basics before merging
- Scan your application after major AI-assisted development sprints
- Have a plan for what to do if an AI tool is compromised
These take hours to implement, not months. Every company that told me "we will do it later" spent more time and money fixing problems than they would have spent preventing them.
What to do instead: Adopt AI tools and security practices simultaneously. They are not sequential phases. They are parallel workstreams. If you are adopting Cursor this week, set up a security review process this week too.
What the best companies do
Across hundreds of conversations, the companies that adopted AI successfully and securely shared a few traits. Looking back on it now, they were essentially practicing FORGE Methodology before anyone had a name for it.
They moved fast with clear boundaries. They did not wait for a perfect AI strategy. They started with constraints: these tools, these teams, these data types, these review processes. Then they expanded based on what they learned. That is Guardrails, Skills, and Schedule defined before the first agent is deployed.
They named an owner. One person owned the AI adoption effort. That person had authority to say yes and the judgment to know when to say "not yet." That is clear Agent assignment at the process level.
They treated security as a feature, not a blocker. Security was involved from the beginning, not as a gate but as an enabler. "How do we do this safely?" instead of "should we do this at all?" That is Guardrails built into the process, not bolted on after.
They created feedback loops. Monthly check-ins on what was working, what was not, what new tools developers wanted to try, and what security concerns had emerged. AI adoption is not a one-time decision. It is an ongoing process. Those check-ins are Guardrails in action: scheduled human review points built into the workflow.
They brought in outside expertise. Not because they were not smart enough to figure it out themselves, but because someone who has seen 200 companies navigate this transition can spot the pitfalls faster than someone seeing it for the first time. A two-hour conversation with an experienced advisor saves months of trial and error.
Where companies are today
We are in a window right now where AI adoption is moving faster than AI governance. Most companies have developers using AI tools with no formal policy, no security review process, and no clear ownership of AI strategy.
That gap is a risk, and it is widening. The companies that close it now will have a significant advantage. Not just in security, but in speed. A team with clear AI guardrails moves faster than a team that is second-guessing every AI-generated line of code.
The patterns I described above are not theoretical. They are things I saw repeatedly, across industries, across company sizes, across technical maturity levels. They are predictable and they are preventable.
